RA: Today there are many treatment options for RA. Many of the drugs do lower the immune response and may predispose to infections such as TB. Nevertheless, rheumatologists are able to mitigate these side effects even in patients with latent TB. We sometimes recommend anti-inflammatory drugs such as ibuprofen for flares. Other Rx: Even though tnfs did not help, there are other biologic agents which have shown good results. One drug is Kineret (anakinra) which works on il-1 (interleukin-1) instead of tumor necrosis factor (tnf). For systemic inflammatory conditions as you apparently have, I suggest one tsp per day orally. It tastes terrible. Something systemic?: Bilateral knee pain and thyroid problems doesn't ring any bells but at your age it does suggest you be worked up for something systemic going on. I would look for markers of general inflammation in your blood and might even consider looking for an auto-immune etiology if nothing apparent on physical exam.
