Somewhat difficult: To imagine what might be causing episodic swelling of the neck. Possibilities might include salivary gland swelling or angioedema. Lymph node swelling or enlargement wouldn't be expected to be transitory. Imaging, ultrasound, CT or MRI, should certainly help here and, especially since this has been going on over a year, you should see your doctor SOON for evaluation. Good wishes:) ...Read more

See doctor/dentist.: You likely have an infection in the mouth (dental caries, gingivitis etc). The tongue swelling does not cause stiff neck which may be a sign of meningitis. You need to check with your doctor if the stiff neck worsens in the absence of other explanation. ...Read more

Follow up: I would recommend you follow up to be seen directly. Although there are multiple possibilities, one of the more likely findings you are describing is lymphadenopathy, in which the lymph nodes are abnormally seollen often related to an infectious or inflammatory process.
