Normal: With peroneal tendon repair, there is most likely suture and/or a graft that was used. It takes a long time for it to absorb or incorporate to your tissues--scar tissue can also cause pain/swelling for a while. I would expect swelling for a year or more. GOOD POINT: That's a good point. A retrocalcaneal spur should hurt directly behind the heel bone. Talk to your doctor again; make sure he/she knows exactly where the pain is located, and ask them this same question. ...Read more

Probably not: Sprains and strains are invariably accompanied by pain. Painless swelling can have many causes . If it is exercise induced it could be a nearby joint capsule, tendon sheath enlarging. Some people also have some small defects in the fascia (tissue that forms compartments in the leg) and muscle can push through these areas during exercise.
Bottom line, if it is not painful I would not worry.
