Smoke and E cigs: As a physician the only advice I can give based on your question is to have you stop smoking all together and to see if the throat symptoms clear.

Partial Rx GERD: I suspect the discomfort is related to your partially treated GERD. You are already taking Nexium (esomeprazole) and using Nasonex. Other non-drug treatments include not smoking, avoiding trigger foods, avoiding large meals, not lying down for 2 hours after eating, elevating head of bed, losing weight, avoiding tight belts, and chewing gum. If you have tried all these, review with doc alternate drug treatment

Use of antibiotics can reduce symptoms and the patient is considered non-contagious within 24 hours. Gargle with warm salt water. Drink plenty of water. Little bit of ice cream for throat pain.

Need antibiotics: For significant sore throat with swollen tonsils / lymph nodes – see physician & get a streph throat test. If you have streph throat, antibiotic therapy is imperative to prevent future compications. Consider tylenol (acetaminophen) or nonsteroidal anti-inflammatory medications. Saltwater gargles: mix 1 tsp salt in 8 oz warm water – gargle several times a day. Or gargle with aloe vera juice twice a day.
