Depends: Strep a has been universally sensitive to the penicillin drugs & treatment within 9 days of first fever stops progression to the antibody production that can cause rheumatic fever. Sometimes total eradication from the throat is difficult but not considered a true failure. Erythromycin is one med that has seen true strep resistance in some parts of the world. ...Read more

Penicillin still #1:
Despite being the oldest antibiotic, it is still the medicine of choice for treating strep throat infection. It is not 100%, but still the "standard of care." it is sometime not used because of penicillin allergies or because of younger age (adolescent/teen-more difficult with schedule--school/sports etc) due to required frequency of use --4x/day for 10 days.

Throat and 'cold": Strept throat only hits the throat, if congested + cough then it i am viral syndrome. It can be very severe but antibiotic will not only not help while it can hurt by causing un-necessary drug resistance. Each wrongful use of ABx can lead to drug-resistance.
