These are very common. They are often fluid filled cysts from scars in the tubing in the scrotum that come with age and life experience. We usually leave them alone unless there is a chronic pain to them.

Intermittent torsion: Is a possible explanation, could be confirmed if you twist testicle counterclockwise till painful. Probably intermittent torsion if twisting exactly reproduces pain. I had gono and was treated with 500mg ceftriaxone iv, 3 weeks ago. All symptoms gone except intermittent small pain on my left testicle. Shld I worry?
Gonorrhea: 500 mg of Ceftriaxone i/v seems like a little too excessive vs. 125 mg i/m as recommended in the us. The pain in the testicle may be residual, but if not gone away within the next few days, you will need to go see doc.
