Why am i not responding to ppi's? Have burn sensation in stomach that leaves throat red & raw. Tried prevacid, (lansoprazole) prilosec, Pepcid ac, gaviscon.

?endoscopy: Gerd can burning in throat and chest discomfort as well as causing food to get caught in esophagus. Costochondritis is inflammation of junction of ribs with cartilage in front of chest along the side of sternum. Usually tenderness to touch and pain with movement but not burning in throat. There is no one best: medication for GERD. the best med will be the one that you & your doc find helps you the best, with the fewest side effects. ranitidine is an H2 blocker, as is pepcid; while nexium (esomeprazole) is a "PPI" or proton pump inhibitor. it seems unlikely, that if a PPI + an H2 blocker is not fully effective now, that an H2 blocker alone (ranitidine) would be effective. review ur sxs w/ ur MD to find the best tx 4 u! ...Read moreSee 2 more doctor answers

Maybe not GERD...: To expand on dr. Dugan's prior answer, evaluation is key to determining if your symptoms are reflux-related or not. Also, if reflux, is it acid or non-acid material that is involved? Sometimes, "sourbrash" (as you describe) may be due to pulmonary, ent, or dental issues. GI care: It seams that you are being evaluated for these conditions. How long on Protonix (pantoprazole) and carafate and how much benefit? Are some of your symptoms newer since on this regimen? No: This is either something else, or you didn't have reflux to begin with. I see a lot of patients who are put on ppis for upper abdominal pain, told they have reflux or an ulcer, and when the pain doesn't improve they finally get some tests and it turns out it was their gall bladder the whole time. Try eating pizza for dinner, then ice cream for dessert. If that brings on the pain, it's not reflux! ...Read more

NEXIUM (esomeprazole) =GOOD: I assume you had a 'scope' (an egd) to diagnose the 'ulcer'. An esophageal ulcer (gerd ulcer) and hiatal hernia can be painful conditions. Nexium (esomeprazole) is a powerful acid blocker (ppi) and an excellent choice to treat 'too much stomach acid.' lifestyle changes will also help: cut caffeine, alcohol and smoking. Can take zantac (ranitidine) or something else while trying to wean off prilosec? Prilosec not helping at all anymore. Have gerd & stomach polyps recent upper endo
Yes: Zantac (ranitidine) has a totally different mechanism of action than Prilosec. It is safe to take up to 150 mg twice a day. Starting a lower dose of 150 mg once a day may be advisable and if this is not enough to control your symptoms, you may increase it to 150 mg twice a day.
