Follow-up study: If you haven't already done so, consider getting another sleep study done with the CPAP machine in place. Endocrinologist need: OSA and elevated TSH levels can coexist. Based on current literature evidence, it is difficult to say which comes first; the disruptive effects of OSA on the hormone system and then subsequently TSH elevations or perhaps vice versa. Other causes: Alpha wave intrusions have not been shown to be a problem by themselves, and do not need treatment. Assuming the CPAP is set correctly, controls the apnea, and you are using it every night all night, I would look for other causes of sleepiness (including narcolepsy and idiopathic hypersomnia). A few rule out tests: I would suggest keeping a sleep diary of how much sleep you get for 2 weeks, repeating the sleep study with your CPAP on then performing a mwt (maintenance of wakefulness test). This will determine if you're getting enough quantity of sleep. You should also have blood work done that rules out hypothyroidism, anemia, kidney or liver disease. See sleep doc: Perhaps your osa is mild, and you've lost weight, and don't need pap anymore, or you're sleeping on your side and had positional osa on your psg. There are many reasons this could happen. See your sleep doc and he/she can look at your study and get your history and provide a likely reason for why this is happening. Recheck pressures: It is possible that the pressure is set too low-daytime fatigue should improve. There are a number of ways to check. Repeat the sleep titration and test the pressure to be sure apneas are gone. Read out the memory from the CPAP machine you use and show to the doctor to see if it is operating properly.
