Get examined!: At age 55 your most common cause for this pain would still be 'impingement' ( or bursitis). Labral tears causing this pain are less likely unless associated with the biceps 'anchor'( SLAP) lesions -- but less likely in isolation in your age group.Osteoarthritis can cause pain in this distribution . Cervical nerve : this would suggest irritation of a cervical nerve perhaps C6 or C7. Your neurologic evaluation should help clarify that. Possible causes: There are a number of entities that can cause the symptoms that you are having. It appears that a work up has been started (negative mammogram). One thing that comes to mind with pressure that increased with leaning forward would be pericarditis, which is inflammation of the sac around the heart.
