No STD, not serious: This could be a prostate problem, with referred pain to the testicle. Or no disease at all; not all pain indicates a health problem. With no clear diagnosis after all this work-up, perhaps nothing more can be done. But it seems clear there is no serious problem and for sure no STD or other infection. Not epididymitis: Infectioius epididymitis almost always involves only one testicle, with marked swelling and tenderness. And the only STDs that cause it are gonorrhea and chlamydia, so your negative tests rule it out. Pain in both testicles without swelling, and/or pain referred to tip of penis usually means chronic pelvic pain syndrome, similar to non-bacterial prostatitis. Recheck with doctor: You'll need to ask the doctor or clinic treating you. You're close to the age (35) after which most epididymitis is not an STD. However, most of those have positive UTI tests. I have to wonder if you had something other than epididymitis (prostate problem?), and lack of clear response to doxycycline suggests something other than bacterial infection. Good luck! ...Read more

Prostate? CPPS?: These are atypically mild symptoms for sexually acquired epididymitis. The only STD causes of epididymitis (gonorrhea and chlamydia) should have responded rapidly to levofloxacin and should have had positive tests. My guess is no STD and probably no epididymitis. Not likely: Based upon your information it seems as though the low back pain and pain running down your leg may be related to the same orthopedic process for which your doctor might order an MRI.
