Go to ER: Considering your age possibilities include cardiac origin. Muscle strains won't hurt you, but cardiac events can. Best to have this type of pain checked out immediately. If you also have shortness of breath, call 911.See 1 more doctor answer
Muscular: In all probability it is muscular, however, if it is a long term or re-occurring problem, I would at least ask my doc for a chest x-ray.
Scapula Pain: Inflammation can occur underneath the shoulder blade where there is a bursa. This is called scapulothoracic bursitis and can be treated with therapy, NSAIDS, pain medications, and possibly a steroid injection. Make an appointment with an Orthopaedic shoulder surgeon to be evaluated and discuss treatment options. Take care, Joe Wilson.

No trauma?: If you have not experienced a trauma, then the likely etiology of your pain is an irritated nerve in your neck. Anti-inflammatories and a formal course of physical therapy is the first line of treatment. An examination with an orthopaedic surgeon including xrays of both your neck and shoulder can help confirm the diagnosis.

A few options: Sharp pain in the periscapular area can be related to myofascial (muscle) strain, primarily rhomboid, serratus, and infraspinatus muscles. If the pain radiates toward the front, consider intercostal neuralgia (an irritation of the nerve associated with each rib). If the pain is at the upper shoulder blade radiating out toward the shoulder, it may be a suprascapular neuralgia.
