Enlargement of the right ventricle and atrium are much better seen with an echo than a CXR. Neck/back pain: Lung cancer is unlikely. The symptoms are more likely due to muscle/tendon strain or a disc herniation in the neck or mid back area. Smoking dries out the tissues and can accelerate arthritis. Smoking slows healing from muscle strains and tears too. Depends: It's hard for me to imagine the pain of a collapsed lung ( I have not have one) but i can imagine the sob. In sinus problem you cough and can cause pain, even rib fracture which was ruled out by xr. I can also cause sudden pulls on the intercostals, hence chest wall pain.
