See your doctor: Skin infections are most commonly caused by S. aureus or "staph." The resistant form is known as MRSA. Antibiotic choice depends on the patient's other medical problems but Cephalexin is a common choice for the non-resistant form and Doxycycline and Linezolid are the most effective oral antibiotics for MRSA. Your provider can culture the wound to tailor antibiotics specifically to the infection.

Spider bite: Seek medical care asap if you think a spider bit you ; you have severe pain (can be felt at bite site ; possible chest, back or abdomen); abdominal cramping /rigidness or ulceration of skin at site. You may need a tetanus shot (if you don't know when your last shot was or it has been > 10 years). Also medical care if you develop signs of secondary infection. Clean area w antibacterial soap.

Depends on symptoms: Some sources say a hobo spider bite is like a typical bug bite while other sources say there are bigger symptoms and some dead skin at the bite site (like with a brown recluse). Some sources believe there are too many cases of mistaken identity with hobo spiders, so it's not known if real hobo spider bites are that bad at all. A doctor can evaluate, if the bite is any worse than a simple bug bite.

If it looks like it is getting infected (increasing redness, pain or not improving) you should see your doctor. Only a small minority of spider bites cause infection or serious skin injury.

Depends: Different spiders cause different reactions. For less toxic spider bite, which are the most common, it usually takes 7-10 days. To decrease pain and swelling, I have found the use of orally administered steroids to lessen pain and swelling.

Several treatments: Recluse bite treatment ranges from essentially local care with cleaning and topical antibiotic ointment to taking oral antibiotics to requiring surgical treatment, hyperbaric oxygen treatment and other more aggressive treatments. There are degrees of envenomation and local tissue response. If you are concerned or if in a critical location (face, genitals, around eye, etc) see md immediately.

If there is redness: If the area becomes more swollen, red or painful it is likely you have an infection and not just a spider bite. We often see patients in the ER with "spider bites" which turn out to be infections from hair, pimple or boil these often grow MRSA or resistant Staph infection so it is better to get it checked sooner rather than later if it is looking red or infected. Usually treated with Bactrim (sulfamethoxazole and trimethoprim) or

Herpes Zoster?: I have seen many patients present with unilateral eruptions they "thought" were "spider bites" or "poison ivy". I would see your eye doc as this sounds like shingles. Herpes zoster ; rx would be indicated.
