Increase Fiber: This sounds terrible for you. Chronic constipation can result in anal pain and spasm. Ideally, increasing your fiber by eating a lot of vegetobles -- I recommend a pound per day -- will soothe your constipation and provide your daily dose of fiber. Vaginal fissure : Are not related to anal fissure. Anal caused by the sphincter too right and tearing when you have bm. Sphincterotomy can be effective, but would reccomend stool softness and topical nitrate creams first, followed by Botox injections if that doesn't work. Sphincterotomy is last measure because of risk of incontinence. Vaginal tears are caused by trauma, especially if there is little lubricants. ...Read more

Spasm: Absolutely, spasm can reduce blood flow and cause pain/aching in the leg. Not sure why you had the steroid IM shot though.
