Probably: Radiographs are not a crystal ball... they don't disclose everything. Did your dentist check to see if the tooth was vital (alive)? Swelling may be from a periodontal issue, so yes, in that case root canal may not be needed. More info necessary to make a proper diagnosis. Tender gums: This is not normal, and it could be an unhealed infection from the tooth, a gum infection, a fractured tooth, etc.

If the infection is too large, if there is not enough sound tooth structure or if the tooth has severe bone loss, you will lose the tooth anyway even if RCT done.
