It takes a toe nail about a year to grow, it could be you had a tiny spicule of nail regrowth. Without seeing or feeling, it is possible. This can be a common reoccurrence 1-3% chance.

Yes: If your doctor says its 'healed' its ok to start exersising again. It may still feel a bit tender and parasthesias (pins/needles etc.) normal. 2: recurrence from its root (matrix);
3: possible injury to the toe during healing period can delay the healing;
4: maybe a low grade infection is causing persistence of the pain.
