Might be an error.: Sometimes, radiology reports have typographical errors in them. "Small paraspinal soft tissue" doesn't make a lot of medical sense, unless your muscles are small because you have small stature or not much muscle mass. Real vs artifact: Did you discuss with the MD who ordered the X-ray? Why was it ordered in first place? Lytic lesions have to be evaluated farther. First repeat the film make sure it's truly there.
