Probably not herpes: Herpes doesn't cause this sort of problem, especially when it lasts several months. This sounds like garden variety fungal (yeast) or nonspecific balanitis, very common in uncircumcised men. Probably not caused by hand sanitizer, even if that was just before the problem started. Benign papules : Pearly penile papules are like a minature "string of pearls" around head of penis. More common or prominent in uncircumcised men. They are perfectly normal.
