I went in at 8 week 1 day for an ultra sound appt baby measured 6 weeks 5 days with a small gestational sac. Hr was 139. is this a bad sign?

At such an early stage, imaging and interpretation is hard.
There are associations between size and abnormalities.
But it is impossible to look at a minimally smaller size and conclude anything, especially chromosomal disorders.

Without seeing the ultrasounds it is a little confusing. At the first U/S, what was measuring 6wk? > the sac or the fetus, if they saw one. The second U/S would actually be reassuring; they measured a fetus at 6w6d (the difference from LMP would just mean your dates are off), but a fetal heart rate of 154 is reassuring. Not enough info: Unless the exact date of ovulation is known, one ultrasound should not provide a prognosis. Also, the sac in the uterus needs to be confirmed gestational as opposed to an ectopic pregnancy. At a true 6 weeks gestation, the sac should also show a yolk sac, fetal pole and cardiac motion.
