My urologist told me small epididymal cyst (measuring 4 mm by 5 mm) cannot be the cause of my chronic scrotal pain because cyst is small. Is he right?
Yes: A cyst that small is very unlikely to be the cause of pain.

Pain in epidydimus?: You've had it for over a year so it must be tolerable. You can have a repeat U/S to see if it has grown in size. Go and discuss your situation with a really good pediatric urologist.

Leave them alone if: The finding of epididymal cysts is usually an incidental find on evaluating scrotal pain or ache with US. Most of them are located at globus major (head of epididymis), clinically asymptomatic, and require no active attention. So, leave them alone if being mentally & physically comfortable. Here are...: It is a common round space of variable diameters from few mm to many cm incidentally found in scrotal US for evaluating some pain / ache in the sac; it is usually located at globus major of epididymis. Such cyst may present as single or multiple like a cluster. More detail? Ask Urologist for detail.

Possible...: Some fluctuation in the tension and size of an epididymal cyst does occasionally take place, although the size of such cysts tends to grow with a widely variable pace over time. Cyst from epididymis: Epididymis is part of the conducting system, and in situated one on either side just above and mainly behind each testicle. It collects the sperms and hormonal secretions from the testes through tiny tubes and is highly convoluted structure -which continues as vas deferens -one on either side. Cysts arising from this epididymis usually are small, sometimes multiple. See ur doc to decide trt plan.
Cyst is spherical: Come in various sizes, can feel tender. Is a small tense fluid filled structure, niether hard or soft. Benign ; only removed if large or painful.

What other things can mimic an epididymal cyst on an ultrasound or is an epididymal cyst on an ultrasound definitely always benign?
No such thing as: "definitely always", but in this case you are close. Malignant tumors of the paratesticular structures are rare but not impossible. These would likely grow more rapidly than the common benign cyst.See 3 more doctor answers
