Difficult: Difficult to say without seeing it, but something that small and unifocal may be a vascular "bud" (capillary plexus) near or part of your taste bud that looks dark to you. Fungal or yeast problems tend to be more diffuse.

Not necessarily: There are a lot of things tha can show up as dark spots in the mouth, most of which are rather benign. It definitely requires follow-up however, in the event that this dark spot is something more ominous. Don't lose sleep over it, but definitely follow up as your dentist recommends.

Many things: You can get a freckle (melanotic macule), a mark from filling metal if you have dental fillings (amalgam tattoo), or something more worrisome. You should visit a dentist or oral surgeon for an examination and possible biopsy.

Talk with an ENT: Biopsies at this age can be very hard to do. I would recommend having a little sedation in the operating room, so that the experience is not traumatic for the child and it is doe safely.

Definitive diagnosis: Is made by the biopsy. If the recommendation is for biopsy - do not delay. Hopefully, it is nothing. In he event it is something, you need to find out.

Black hairy tongue: There are a number of ailments that can lead to black pigmentation on the tongue. Black tongue: Could be normal in dark skinned individuals however sudden changes could be due to infections; yeast, medications including antibiotics or inadequate oral hygiene & less commonly, due to other health conditions. Try brushing the area with soft bristle tooth brush / toothpaste at least twice daily, if persistent or if other assoc. Symptoms, get him evaluated. Take care.

Tongue lesion. Started about 5days ago. Thought it was from drinking hot beverage, but not definitive. Turned from red to white with black spot under.

Normal healing: This sounds like you are going thru the normal healing process after a burned tongue. It will take about 7- 10 days to heal. If it does not heal, you have pain, or the size of the area is increasing you should see a dentist asap.
