Hard to know: Spotting in early pregnancy is common and not necessarily a sign of miscarriage. If you are only 4 weeks pregnant, the urine pregnancy test might not show up positive yet. If slow upward climb, or plateau, or drifting down, then unfortunately, yes, miscarriage likely.
