Too late for vaccine: At age 45 + the presence of HGSIL already, it is too late to be immunized with the HPV vaccine which is recommended for ages 11 and up for both boys and girls. After the coniization, you will need regular follow up care with your GYN to avoid recurrences and Rx if you do have them. By the way, be very careful about future sex partners by screening them carefully pre event. ...Read more

Pap and HPV: HPV associated with an abnormal pap smear is obviously a greater concern then one without HPV due to certain HPV strains ( 16 & 18) going forward to cervical cancer. Conization of cervix: After the procedure, you may have some cramping or discomfort for a week or so. For about 4 - 6 weeks avoid the following -douching (you should never do this in any case), sexual intercourse, using tampons. For 2 - 3 weeks after the procedure, you may have discharge that is bloody, heavy, yellow-colored if heavy bleeding or foul odor notify your doctor. 2 schools of thought: There are two schools of thought in regards to hpv. One school believes that your immune system "contains" the virus and as such you may have no symptoms. But, if things change, the virus can "temporarily escape" and you become infectious. Another school of thought is the belief that the virus can be completely iradicated from the body. Depends on…: Whether conization eliminated all of virus. Was follow-up done to check for presence of more virus? If no virus present, u can't pass what doesn't exist. If any virus still present, then it can be passed. Virus can also exist in mouth, throat, anus—areas that also need to be checked. ...Read more

Biopsy diagnosis?: Until the cervix is biopsied via Colposcopy or other means, the Pap smear diagnosis is irrelevant. We use the biopsied tissue to determine and document the diagnosis. Future pregnancy is possible if it is not invasive cervical cancer and even then eggs can be harvested from your ovaries prior to definitive surgery. ...Read more
