Hard to say: The shoulder pain doesn't fit well into the er doc's diagnosis. Ear pain can come from eustachian tube dysfunction due to irritation at the back of the throat. Upper back: This sounds like a muscular spasm or strain brought upon by the deep breath. If the pain was very sharp and more to the side rather than the midline, a pinched intercostal nerve or muscle (runs between 2 ribs) is another possibility. Need more info: How long have you had this pain? Do you have any fever or sore throat? it could be a flu type of viral infection. If it is viral, it should settle down in 3-5 days.
