Intrinsic shoulder: Joint problem, such as rotator cuff tear or frozen shoulder joint may be present. The numbness could be indeed "shoulder hand syndrome" or maybe brachial plexus issue associated with the shoulder pain. Perhaps a parsonage turner syndrome or even a thoracic outlet syndrome if symptoms worsen on arm elevation. However, sounds like a mix of joint and plexus. See orthopedic surgeon first. ...Read more

Probably referred: I would first look at your neck as the root of your shoulder pain and the numbness you are having. The fact that your feet and hands are going numb are a bit concerning espeically the feet. This could represent a central cord compression at the level of your neck. EMG should show: If you had a normal cervical spine MRI it is no surprise that epidurals were ineffective. The EMG you said is abnormal - it should suggest the nerves affected and allow your pain management physician to develop a diagnosis and treatment options.
