Thorough exam: I would recommend seeing a doctor very soon - perhaps start with an urgent care. The history you give is a little unclear but the combination of SOB, chest pain, throat tightness, lightheadedness and fatigue needs to be evaluated ASAP. Go today.

See your doctor: Chest tightness and shortness of breath with exertion could be angina or pain from your heart. It also could be asthma, lung infection or several other things. Your doctor can help sort this out.

I get shortness of breath when standing up. Sometimes I get chest tightness. And very rarely mild chest pain. I am 18 with normal echo and ekg. Is it cad?

SOB after standing: Could be gastritis. Try ranitidine/ prevecid. Normal echo/ EKG eliminates lots of conditions- abnormal heart wall thickness, mitral valve prolapse, abnormal rhythms. Did you have a stress echo test? That may tell a bit more as you see heart response to stress/hyperactivity. Check PFTs for asthma and tilt table test for autonomic dysfunction like orthostasis or tahycardia (POTS). f/u w/doc

Chest Pain: Chest pain from a narrowed heart artery, called angina, typically occurs when you are active or exercising and goes away when you rest. It is often a tightness or pressure or burning in the chest or neck or arm. If you are only feeling symptoms when at rest at night and not during exertion, it probably not your heart, but I suggest you see your doctor.

Go to the ER.: There could be all sorts of bad things going on. I would seek medical attention as soon as possible. Hope it's nothing serious but better to be sure.

Broad range: It is hard to know without examining you first but this could be anything from anxiety to asthma attack, pneumothorax (air trapping the lung), or infection. I suggest you have it checked out with your doctor very soon.

Probably not: If your symptoms are chronic in nature, the er is not the best place for you to be evaluated. Go back to your regular phsyician, who knows your entire story. Ask if there is any other specialist who might be helpful in solving your difficult problem.
