worse with cough?: Does it get worse with cough? Deep breath? Fatty meals? Lying down? Arm movement? It could be many things but a muscular pain is most likely. Gall bladder and biliary tract disease should be considered. Pleurisy is also a possibility. Cervical spondylitis is another consideration. Shoulder arthritis or tendinitis. Rotator cuff tear. Symptoms: Sounds like you may be an air swallower and have gaseous distension. You could try 'Gas-X' tabs, 2 QID for a few days and see if it helps. If so you need to learn how to swallow less air. ...Read more

Back trauma?: Shooting pain is often neuropathic pain, which is often associated with a secondary muscle spasm. (cardiac or noncardiac is open until the diagnosis is established!) neuropathic upper back and chest pain does happen (i.e., in thoracic nerve roots disease, aka radiculopathy), although it is not very common.
