Many possible: There is a very small muscle in the calf that is called the plantaris muscle and sometimes it can get injured while working out. People often describe a sudden sharp pain in the calf. My best guess given the supplied info is either a pulled or partially torn gastrocnemius (resulting in a palpable lump) or possibly a soft tissue mass giving you nerve pain. Need exam...: This is obviously difficult to answer w/o an examination. Gastroc muscle strain from 2 yrs ago should be healed by now unless u keep re- injuring it and could be missed possibly on MRI. In my experience, "SI joint" and "Piriformis" syndromes are commonly over-diagnosed by mistake.
Instead, I do see a lot of cases of sciatica from degenerate discs and pinched nerves in the lumbar spine. From your description, I wonder if you might have a pinched L5 nerve in your lumbar spine.
