Intermittent torsion: Is a possible explanation, could be confirmed if you twist testicle counterclockwise till painful. Probably intermittent torsion if twisting exactly reproduces pain. This needs eval: There is really no way you can figure out what this is without an examination. It could be painful and swollen due to trauma, fluid build up (hydrocele), inflammation (orchitis), or many other causes. ? Intermittent torsio: Intermittent torsion (it) should be ruled out. This where the testicle twists on the spermatic cord and first cuts off the venous return. It is not very common, but, you can check yourself by slowly twisting the testis in wards (like closing a book) and keep on twisting until it is painful.
