Lots of things: Most common back pain sources are strained/pulled muscles but "sharp" pain into the shoulders and arms/legs can be from a herniated disc causing nerve compression. If the pain comes around your chest/abdomen from the back this can also be a disc herniation. Why when i'm breathing I have sharp pain in my upper back on the left hand side? Also i'm breathing a lot more heavily with tightness in chest why?

Go to doctor: Anything that bothers you this much should be worked up. You should see your doctor so at least some blood test, some xr and other tests can be done to find out why is wrong with you. Ot is either a pleuritic chest pain or nerve pain from muscle stretch but the tightness in breathing should not be there unless it is from the pain. Still you need to be examined.
