Pleuritis: Sharp pain in the left shoulder and chest and extending all of the way down the left arm in a 44-year-old should at least be evaluated to rule out cardiac ischemia. If you are not having any problem with your heart then likely you have a pleuritis, a costochondritis, a rotator cuff tear or a cervical radicular pain. Muscular Pain?: Hi, your recent onset of symptoms and your age suggest that at this point the most likely diagnosis is that you have a shoulder/upper back/neck strain. You don't describe radicular symptoms that would suggest a neurologic etiology. Initial treatment after an H&P and imaging usually starts with NSAIDs and maybe a muscle relaxer and also some therapy exercises to see if your symptoms will get better ...Read more

In broad overview, sharp, short pains are less often of serious cause unless pain is quite severe and/or associated with other abnormalities. See below: You describe pleurisy or pericarditis. If your pain is relieved sitting forward, worse lying on your back, you likely have pericarditis. If not, it's more likely pleurisy. These potentially can be serious symptoms and should be investigated. Do you use birth control pills ?(cause blood clots); do you have fever? (may have pneumonia).
