Varies: There are several possibilities including tendinitis of the patellar tendon at its orgin on the patella or chondromalacia on the under surface of the patella. Early wear and tear: Pain coud be due to irritated/early wear of the patella cartilage or the surface cartilage of the thigh bone that rubs against the patella (try knee sleeve with a cutout for knee cap). Other causes could include patellar tendonitis (try knee strap) or a plica and osgood schlatters. For starters avoid kneeling, stairs, knee extension machines, deep squats, lunges etc. Try therapy, ice, advil/alleve. ...Read more

Need X-ray : If you haven't done so already, you need to be examined and have an x-ray. It is possible that you have fractured/broken your knee cap which sometimes requires surgery or at least a brace or cast. If not, then most likely you have sustained a contusion or bone bruise which should improve with time. Several thoughts: Pain below your kneecap area which is the patella tendon can be caused from inflammation of the patella tendon. It can also be from arthritis under the kneecap. There are also skin disorders and bursitis that can cause pain in this area. You really need to get a good physical examination to be able to make the true diagnosis. Out of the blue: Problems like this frequently come "out of the blue" meaning for no obvious reason maybe early chondromalacia of the patella try hot and cold packs both work well for non traumatic problems may also wrap knee with plastic wrap at bed time to limber it up and keep in body heat if not better in a few weeks then it needs attention of a doctor or more specfically an orthopedic surgeon. ...Read more
