Have had physical exams and nothing shows up, . Next, look at what creates anxiety. Sounds like you might have chronic anxiety, which would explain your symptoms continuing beyond a day or two. Neuro exam, EMG, MRI of brain normal. No spinal MRI done. Weak, shaky legs and dizzy a lot. Off balance. Fatigue, fascilations. MS? GAD? Neuro?

It's tough to explain the cause of your symptoms, especially with a normal MRI of the brain, neuro exam and EMG. What about blood tests? What about other symptoms? There is probably a missing clue somewhere. It's not clearly in the neck.

Considering all your symptoms and negative exam and other tests like MRI, EMG and blood test. It is more likely to be due to anxiety.
But I see you are getting behavior therapy, and Cymbalta (duloxetine) which is used for Depression/generalized anxiety disorder
