The radiologist who interprets the images will know if the spots are worrisome, and will then order a different test or a biopsy. It depends: Most findings are benign. However, you need to ask your doctor what these mean - Do they appear benign? Are they worrisome for cancer? What are they (cysts, adenomas, etc)? Not enough info: Without seeing the actual ultrasound or at least the ultrasound report, it would be difficult to figure out what is meant by a "shadow" in your pancreas or liver.
