Other: Other things to consider if this happens routinely after you eat are gallstones and inadequate blood supply to the bowel. Gallstone pain is classically brought on by eating fatty foods and is most easily tested for with ultrasound. The problem of inadequate blood supply to the bowel (called mesenteric ischemia) is much less common than gallstones. It can also be tested for with ultrasound, though the tech needs to know that he or she should be looking at blood vessels.

Tear or acid: Could be your esophagus, a tear (Wallory Wies) or acid aka GERD. Get evaluated by your doc, ask for scope aka EGD. This procedure will make the diagnosis and get you started on treatment. Okay for TUMS (calcium carbonate) in the meantime. Be well.

Morning pain: A peptic ulcer is suggested as the discomfort appears to occur when you've gone hours without eating and the distress is improved after drinking milk. Duodenal ulcers are a common occurrence. I recommend that you consult a physician or a gastroenterologist for diagnosis and treatment. Appropriate tests will be ordered.

Uncertain: It sounds like your next period is not due for some time. This is not necessarily a gynecological issue. If you are experiencing severe abdominal pain would recommend medical assessment.

Needs workup: Try to make an appointment with your primary care doctor or GI doctor but in the meantime, write down if there are particular drinks, foods, or medications that bother your stomach. Also keep a list of every medication and supplement that you take for your visit. It sounds like you need either a scope or a scan.

Many possibilities.: Please keep working with your doctor to find a cause and treat the problem. Any list of possible diagnoses would be guesswork, however a very detailed history of when the symptoms occur and the events surrounding them may provide clues for your doctor.

Gallbladder: The first that comes to kind is gallbladder, but it could also be ulcers. You should see a doctor as soon as possible. This could be easily solved bit waitron could complicate things.

Constipation?: 20 y male seems to relate symptoms of recent-onset constipation. Opiate pills, dehydration, lack of roughage & Irritable Bowel Syndrome, constipation type, might be explanatory; perhaps has recent dietary changes. Fluids, fiber, prunes & a touch of OTC laxative might resolve. If problem is persistent, see doctor. If have constant/severe abdominal pain &/or fever/vomiting obtain urgent evaluation.

Seek Help: You will likely be best off seeking urgent medical attention, given the description of “severe” abdominal pain. This pain may or may not be related to your menstrual cycle, mentioned in your question. In other words, the differential diagnosis of the pain that you describe is quite broad. Best wishes.

See Your Doctor: Nausea is a common side effect of Abilify (aripiprazole). Severe abdominal pain is not. If the pain is more than just momentary, you should definitely see your doctor or go to the Emergency Department if unable to see your doctor.

Most likely PID: Painful intercourse, also called dyspareunia, is often related to pelvic inflammatory disease. That said you have lost weight. Was that unintended? There are many possibilities that come to mind and in this instance I suggest a visit to both your primary MD (family or internal medicine) ANDa visit to GYN. They will need to work together to diagnose and treat this problem, ASAP. Delay is unwise.

Go to ER: Your pain sounds very much like PANCREATITIS the change in position is the clue! Other upper abdominal conditions include stomach ulcer (s) and Gallbladder inflammation... Hope this helps and good luck! Dr Z
