Endoscopy: See a G.I. Doctor to get an endoscopy. Could be gastritis, an early ulcer or an esophageal stricture. If any of the above, it is likely caused by an infection with a bacteria called helicobacter pylori and can be treated with antibiotics and acid blocking medications. Many possibilities: Pain in that area could be due to trauma (fracture, tendon or ligamentous tear, arthritis), infection, inflammation, metabolic conditions (gout) musculoskeletal (bony over growth) or biomechanical abnormality, or neurologic conditions (neuropathy). Go to the ER: Severe chest pain should never be taken lightly. The positional nature of the chest pain makes one think of a possible pericarditis (inflammation or infection of the lining around the heart) although this could be a heart attack, blood clot, lung infection, etc. All possibilities that are very serious and should be excluded rapidly. Costochondritis: This is an inflammation of a joint between the breast bone and a rib causing pain and tenderness. If the nerve running between 2 ribs is also pinched, sharp shooting pains could radiate around to the side and back of the rib cage.
