A : A board certified oral and maxillofacial surgeon, in addition to an ent, would be a good choice since they can evaluate issues within the mouth and upper throat. Burning mouth syndrome is usually due to issues within the oral cavity, but discomfort can extend down the throat.The fact that you have a hiatal hernia and are being treated for reflux is a significant clue to the problem. Need more info: Need to know do you smoke, drink alcoholic beverages frequently, eat spicy foods e.g. Mexican, post nasal drip and a few other reasons. Talk things over with your family physician. ...Read more

LERD possible: Exposure of the vocal cords to stomach acid & pepsin will result in inflammation of the vocal cords (as is also often seen with acid regurgitation). Acute laryngitis may result, but watch out for chronic hoarseness, throat clearing, airway spasms, bronchitis, an asthma-type presentation, and even a higher risk of throat cancer. Possible: Depends on how big the gland was originally. If it's been a long time since you have it rechecked, now would be the time. It's possible that your current symptom is thyroid related, although unlikely. ...Read more

Not sure: Not sure without having looked at the big picture. But is any viral infection ruled out? In case ct scan misses owing to low quality of image, nasal swab will have to be done. ...Read more

Laryngologist: When the rln is injured, the vocal cord is paralyzed. Be seen by a laryngologist if you havent already. Some injections (dermal fillers) can be done to improve the voice by moving the vocal cord over a few mm. There is s more permanent solution with a procedure called thyroplasty which uses a silastic implant to push the paralyzed cord into a position that improves the voice. ...Read more

Esophagitis: Gastroesophageal reflux can cause espohageal pain, spasm, burning and even hjoarseness as stomach acid irritates the vocal cords. General recommendations include PPI meds (Nexium, Prilosec, Prevacid (lansoprazole) etc), no eating within 2 hours of bedtime, decreased caffeine/alcohol, smaller meals with snacks in between, elevate head of bed abd see a gastroenterologist. ...Read more

Head & Neck Surgeon : You have a goiter that is pressing on your pharynx and even compressing your trachea (your airway). You need to have it removed surgically ASAP! Go see MD (not a nurse practitioner) and don't let them dilly dally around.
