Coronary arteries?: 59yo female reports chest pain with exercise. Physicians generally rule out the most worrisome pathology ascribed to symptoms patients report. Before all other considerations, a 59yo women with chest pain upon exercise needs an in-depth evaluation for angina pectoris and coronary artery disease. If a workup is negative, then consult with an Osteopath. These symptoms require expeditious evaluation.

Not unusual: An MRI arthrogram is preformed usually injecting gadolinium and iodinated contrast. One could have allergy reaction to either. Pain after this study is rarely sever. Talk to doctor who did the study. Infection or blood in the joint could produce pain. Rather than taking a pain med do not overlook the possibility of an adverse reaction. If clear for adverse reaction any mild pain medicate is ok.

No.: How soon after the arthrogram did you begin having severe pain? Infection is a relatively rare but serious complication of arthrography. Call your doctor and explain your symptoms.

Yes and No: You should not use any narcotic medication for joint pain related to arthrograms. NSAID such as motrin or tylenol (acetaminophen) may help. I think that joint pains related to the arthrogram can in part be related to a patient's sensitivity to one or more of the drugs used in the study, but not an allergy as such. Many of my patients get good relief with adding 25 mg of benedryl orally with the NSAID.

Yes and no.: A standard MRI with contrast usually implies an intravenous injection of gadolinium. An MRI arthrogram means that the contrast is injected directly into the joint space, usually with fluoroscopic guidance of the needle.

Using a Tesla 3 MRI s it possible to have a normal MRI of hip and then in the same session have a MR-guided MR arthrography of the hip joint?

Yes: Yes you can have both. MRI contrast is injected into hip joint and MRI repeated. Theory being that if MRI without contrast revealed the problem no further testing needed. If it was normal a repeat MRI with contrast in joint may show abnormalities not seen on the first study without contrast. (Gadalinium)

False negatives: Almost all tests have some false negatives. Depending on the severity and extent of synovitis it may not show up on tests. It also depends on the type of inflammation. There can similarly be false positive test results for most of the tests. Wish you good health!

AS: Ankylosing spondylitis is of concern. A plain x-ray of the sacroiliac joints would pick up images of bone sclerosis better than an mri. A rheumatologist can discern and advise. Traumatic injury to the sacroiliac joints is a consideration. Has there been participation in extreme sports? An osteopath can evaluate injury to the sacroiliac joints.

See details: Have you seen a rheumatologist to be sure you do not have spondylitis. Many people believe they have hip pain when in truth it comes from the lower back. There are very effective therapies for B27 associated diseases.

Let me explain: Some times you have problem inside the hip joint (labrum tear) dose not show on the regular mri, you need MRI with contrast to see if you hip joint pathology will explain your symptoms.

Talk to surgeon/Dr (s: Definitely start w/ the Dr and surgeon involved. Be persistent about their need to help you have the best possible resolution through whatever means they can. It may involve more Rx, PT, referrals or even more surgery - or psychotherapy to help you adjust to a new normal.

Hard to say: One white spot on brain MRI is nonspecific. If it is in a particular area (corpus callosum) it would be more concerning, otherwise it could have been from prior brain infection (encephalitis) or trauma (shearing injury). Your symptoms are a bit more worrisome. Perhaps a spinal fluid analysis will help. Neurology consult would be useful.

Several: Tylenol (acetaminophen) with Codeine is often used. Vicodin (tylenol (acetaminophen) with hydrocodone) is also frequently given. Ultram is a non-narcotic alternative. There are other, more potent medicines that can be given for very severe pain.

Varies: The two most common causes would be a problem in the hip verses a pinched nerve in the back. The MRI arthrigram should go a long way in determining if it is from the hip or not.

Tough decisions: Hip arthroscopy is in its infancy, and we do not have long term studies on patients that we have repaired. Therefore, each hip case needs to be individualized with a surgeon that you trust and have a good rapport with. There are few clear cut answers.

More complex study.: I assume you mean an MRI of the hip compared to an MRI of the hip with an arthrogram. Both studies include an MRI of the hip. The study with an arthrogram helps to better visualize the labral and articular cartilage about the hip. Along with an arthrogram, additional medication is placed into the hip joint, which can help determine if your hip pain is inside the joint and capsule or outside.
