Options available: CT scan of the abdomen may be indicated but not for mild pain alone. If the symptoms are progressive and there is concern for Cancer (unlikely in your age group), then only I would consider doing a CT. Similarly Colonoscopy is not indicated because of pain alone. Melena: Black tarry stool, otherwise known as melena, can sometimes be due to diverticulitis, but certainly can be a sign of bleeding in the upper digestive tract. IBS, other?: This is hard to answer without exam and some work up with labs, etc. However, it sounds like this could be IBS with fluctuating constipation and maybe some haemorrhoids that are causing blood in the stool. Yes: Alcohol intake can cause bleeding in the intestine through multiple mechanisms. Alcohol is a poison and never did anybody any good. Your girl friend would be better served by not drinking. For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. Drink enough water daily so that your urine is mostly colorless.

Colon polyps should be followed with colonoscopy anywhere from 6 months to 5 years later, depending on type of polyp and confidence in that it was completely removed.
