Yes: It can. It would be helpful to know your age and history to comment further. Septated cysts are also a malignancy risk but there are septated benign tumors too. Do you mean 27 cm or 27 mm? Yes: While it is impossible to predict whether or not ovarian cysts/masses are benign or cancer, usually large ones need to be dealt with surgically. More info needed: It would be helpful to know your age. A 27cm ovarian cyst has a low chance of being cancer in a young women in her 20s; however, in an women in her 60s it would be very worrisome for cancer. No matter what your age, you should have it surgically removed. You might consider seeing a Gyn Oncologist for this surgery as they are the most qualified to perform the surgery on ovarian cancers. ...Read more

Pain control/US: Get an ultrasound to make sure there is no ovarian torsion. Finding a balance between pain and daily life is sometimes difficult. Expecting to be pain free is not possible because there is a ruptured cyst. Try acetaminophen and/or NSAID such as advil or aleve. Make sure blood counts are okay. Need more details: I highly recommend you consult your oncology team and get evaluated by a gynecology oncologist. Since you are <50, it makes me concerned about a brca mutations (if you have not been tested, then get tested!). Ovarian cancer and breast cancer can be related, especially if brca is positive. If you have a estrogen positive tumor, then stopping the ovaries from making hormone may be beneficial. ...Read more
