Perhaps: Recommend a visit to a urologist, as maybe prostate issues, causing urinary complaints, but ravages of ms and associated prior urinary tract infections may have culminated in permanent bladder problems. An ileostomy could be considered. Likely, at this point, the ms is "burnt out", and the usual disease modifying agents are unlikely to benefit.
