R/O Fibromyalgia: 42 year old lady with 14 year history of fatigue, low grade fever, cold extremities, joint pain, facial flushing and digestive problems. You may have fibromyalgia, or chronic fatigue syndrome. It is far from being "all in your head" it is a real condition affecting up to 5 million people in the United States according to the CDC. People with fibromyalgia experience extreme fatigue, widespread joint pain without inflammation, digestive problems such as IBS, and several other issues. While it is difficult to entertain a diagnosis such as fibromyalgia with only a presented medical case, I think this is likely your diagnosis. There are diagnostic criteria set forth by the American College of Rheumatology and I would personally encourage you to visit an academic medical center in the Bay Area that is convenient for you. UCSF, Stanford, and UC Davis are all outstanding medical facilities where you can discuss this more. Immune deficiency: Would like to know how low the IgG is . In this case I would consider a functional immune evaluation by studying his pre and post Pneumococcal 23, and Td immune response if the IgG is only borderline low. If it is very low, then IVIG is likely indicated (I have yet had a patient who can tolerate SCIG). I would also like to find out what the urinary culture results are. If they have not been revealing , then a PCR bacterial analysis would be indicated.
