About once every few days for the last decade or more, when I sneeze, I get a real painful cramp for about 7 seconds in the area at the bottom of my scrotum or near the vas deferens i guess.

Could be normal: As we get older, we get more "spots" on our skin, including the scrotal area. Because normally skin here is 'wrinkled-up' and we usually don't check down there daily, seeing some darker spots can be worrisome. Stretch/flatten it out to see what it looks like.
