Had blood patch 5 days ago due to PDPH. Was feeling pretty good until today when headache is back and now I notice neck pain and when I bend my neck I feel low back pain and pressure.

Depends: Hi. If you're obese or overweight, weight loss may lower BP. Regular exercise may help lower BP. Weight loss is easier said than done, however. The most direct and proven means is to take BP meds ("antihypertensives"). Personally, I would never put off treating high BP with the hope that weight loss or exercise might control it.
