Low grade temp: For most, a fever is greater than 100.4. However everyone is unique -from the nose we have to the body temp we keep. I'm not sure 99.4 is really significant. Do you have any other symptoms to go on? If your testing so far has been negative, then I'm not worried about you. Maybe there's something in your routine you can change/improve. Try 8 hrs sleep? Drink more water? Eat more fruit. Feel better.

Maybe: Sometimes a mild fever is of no consequence. If you also have other symptoms, it could be something more serious. If you feel bad in addition to the fever, the symptoms can help a doctor figure out what's wrong. For example, a mild fever with congestion and cough might be a cold, but high fever with neck pain and headache could be meningitis. So the symptoms are everything.

Fever in 13 month ol: A fever in the 100 range is not serious and is pretty common in young children after 6 months of age, when they have lost all the immunity they got from mom, and then get lots of infection. A single spike up to 102.7 is not concerning, but persistent fever at this level would be. As you guessed, hydration is most important and continuing to eat well, in which case you can continue to just observe.

Not likely.: Chills and rigor are synonyms and are seen due to the release and action of cytokines and prostaglandins as part of the immune response which also increase the body's set temperature in the hypothalamus. The person then feels cold with respect to its environment and so has chills. They happen, therefore, during a high fever. Rigor mortis is a condition after death where the limbs stiffen.

Several: Some folks get goosebump, teeth-rattling chills, especially at night, with no fever. When it's not due a cold-water environment or a fad diet, this can be puzzling. Most often a medication side-effect, this can also be due to a bladder infection that produces no fever. Your physician may wish to check you for hypothyroidism and/or early diabetes. Good luck.

Incomplete info: What sputum test? Was it culture for tb? Do you have cough? What other symptoms do you have? Fever of unknown origin is usually a difficult issue to solve and this is not an appropriate forum to address the matter.

During: During a cold a person's appetite is usually diminished. There are no food restrictions per say. Dairy products can contribute to mucous production, so it is good to avoid dairy, chicken is fine. Also make sure he stays hydrated. He should increase his oral fluid intake (water, gatorade). If his symptoms worsen, or his fever persists he should see his doctor.
