If you inserted it after your period ended, then it is best if you use a back up method for one month. This does not necessarily mean you are not protected, yet the odds are greater that you are not since you inserted it late.

Pregnancy unlikely: The second day of your period is not a high-risk time for pregnancy. Even if you missed the day's pill altogether, it would be unlikely that you would get pregnant on this day. Not sure: Exactly what you did, did you skip the placebo pill and started a new pack immediately. If so, you didn't need the morning after pill.

Talk to your gyn, they can work with you on other pill options that can do that all of the time.

Not exactly: Birth control pills take one full cycle to be effective and you should be using other methods (such as condoms) until you have completed the first pack. The uterine lining can get thicker and may not shed completely leading to spotting post-period. See your GYN if your cycles do not normalize after 3 months off the pill.
