General rule: Creams or lotions you apply to the skin surface will not transfer a detectable amount of med through the skin to your blood. The best over-the-counter acne remedy is benzoyl peroxide, but if it's bugging you enough to consider "cold-pressed" castor oil, see if your physician would add topical Clindamycin to peroxide.
