Unlikely for 18: Weakness and numbness in the arm is more likely from as c5 cervical disk then a rotator cuff tear if you are under 40, particularly without major trauma.

Nerve?: The pain can be from many possible sources. Deltoid area pain with numbness may signify involvement of the axillary nerve or possibly the c5 or c6 levels at your neck. Neck shoulder: Have your neck checked. Most shoulder problems can radiate to the elbow but not as far downstream as the forearm or hand.

Not usually: The shoulder is usually an 'innocent bystander' when it comes to neurologic problems such as numbness and weakness that is found beyond the shoulder in the arm and forearm. The more likely culprit for these complaints is actually nerve compression emanating from the cervical spine.

Pinched Nerve: Sounds like you have a pinched nerve in your neck. I would suggest seeing a pain/spine specialist to evaluate you further to evaluate what level and what can be done to treat you. There are interventional treatments besides medications and surgeries that might reduce or eliminate the pain altogether.

T.O.S.: Chronic shoulder aching followed by tingling ; numbness arm ; hand suggests thoracic outlet syndrome, as arm nerves are impinged (pinched) in shoulder region. These symptoms oft occur when arm raised to shoulder height as clavicle (collar bone) caused to rotate downward causing closure of space between clavicle ; underlying 1st rib. See an osteopath. Imo many neurologist doubt existence of tos.

See below: The nerves for the arm pass very close to the site of your shoulder surgery. It is quite possible that some swelling or scar tissue is impinging on them as they cross near this area. Talk to your orthopedic surgeon about your symptoms and let them guide your care.

Cerv Radiculopathy: Pain that radiates into the arm with associated numbness or weakness, suggests a pinched nerve. In older patients, this is often due to arthritis. Younger patients, herniated disc is often the cause. If symptoms result from postures or positions of the arm or shoulder, other causes are suggested: thoracic outlet, cubital or carpal tunnel. For symptoms that persist or worsen have a doc take a look.

Could be: He does have a number of risk factors for heart disease (sex, age, hbp), but he likely has a nerve entrapment issue in his neck. Either way, he should seek care if his symptoms don't improve. Hope he feels better soon!

? ulnar: Your description suggests you may well be compressing ulnar nerve at elbow, but this could also represent an issue at the wrist. An EMG test could reveal location and severity. Contact your doctor and get opinion.
