Pinched nerve: During sleep many times due to position if limbs, there could be extra pressure at nerves to cause a variety of symptoms including tingling and numbness. Make sure you sleep on a firm mattress with soft pillows and change positions frequently to avoid constant pressure. Comments: Fluctuating symptoms can occur, and some of these may involve "ephaptic short circuiting" due to loss of myelin insulation due to permanent damage. This can easily be controlled and treated using some anti-epileptic drugs. Need to exclude superimposed co-morbidity, such as nerve compressions, by EMG study, and check B-12. Possible: Migraine associated vertigo is a well known migraine variant and with a history of migraines this is a possibility. It can be very responsive to medications such as topiramate.
