Nerve: This could be a pinched nerve that is aggravated in certain positions. If you are not extremely bothered by it, there is no need to seek urgent care. If you see rash in the area, then I would see an M.D. as it could be shingles. The tingling with shingles though, is not usually intermittent or position dependent. ...Read more

Get imaging: Could be...I'd get imaging of the Thoracic spine (an MRI). if it wraps around one side and gives you numbness is a classis distribution, it could be a disc herniation or a pinched nerve for another reason. If it's on both sides, i'd get some imaging of this area given your history of duodenal issues ...Read more

Get EKG: although could be musculoskeletal (rib injury, shoulder refered pain, neck problem). you need to rule out cardiac issues even without previous personal or family history...suggest get an EKG as soon as possible.
